Lisa Hogan is a 2017 & 2018 Telehealth Honors winner who operates the telehealth program at Frederick Memorial Hospital (FMH). Lisa led the implementation of FMH's Chronic Care Management Program (CCMP) in 2017. Since then, FMH's CCMP has achieved an 89% reduction in readmissions and a 52% reduction in average costs.
In January, Markets Insider published a press release highlighting the accomplishments of the FMH’s CCMP. Lisa has been featured on Maryland's local WDVM TV to introduce the program and share FMH's results.
